A large proportion of paediatric patients admitted to the PICU meets the SIRS Criteria even though a significant number do not have sepsis. However, it is difficult in the PICU to reliably distinguish SIRS patients who are infected versus those who are not. Clinical and laboratory signs are similar in both sets of patients. Procalcitonin (PCT) is a blood biomarker that can help discriminate between infectious and noninfectious causes of critical illness. PCT has been demonstrated to be the most useful tool for early identification of sepsis in the PICU, superior to other markers like CRP or leukocyte count.
